The primary function of the Nurse Practitioner is to assist the Medical Director/Collaborating Physician by ensuring maintenance of high standards of resident care by expanding access to advanced medical procedures including assessment, treatment, medication and therapy. The position also facilitates the day to day communication between front line care givers and advanced practice professionals and provides for timely, accurate and effective response to changes in resident condition.


Provide direct resident care within the scope of practice of an Advanced Practice Nurse and in collaboration with the Medical Director.
Document and code care procedures to ensure appropriate billing of services provided. Required Skills 1

Graduate of a College of Nursing and Nurse Practitioner program, with current license to practice as a registered nurse and as nurse practitioner in the state of Ohio. Able to prescribe medication, treatment and therapy within scope of practice of CNP.
Geriatric specialization preferred.

2 or more years experience as CNP.

1 or more years experience in Long Term Care as a nurse, clinician or administrator.
Able to read, write, speak and understand English. Must have excellent oral and written communication skills and must be able to create and edit reports, orders and documents using computer applications.

Ability to collaborate with inter and intra disciplinary teams

Excellent clinical and analytic skills. Strategic thought process to permit maximization of funding sources.
Highly organized, able to keep accurate and timely records of care and to maximum service plans and plan codes to ensure all care provided is reimbursable as well as necessary and appropriate.

Ability to stand and walk for long periods of time, use hands and arms with dexterity, occasionally stoop, kneel, bend or crouch. Must frequently lift, move or physically manipulate residents using proper body mechanics. Must be able to communicate clearly and effectively with residents. Visual acuity, including the ability to see at close range, and accurately perceive color required. Requires peripheral vision, depth perception and ability to adjust focus. Must be able to tolerate moderate noise levels and discern conversation in atmosphere of background noise. Required Experience

2 or more years experience as CNP.

1 or more years experience in Long Term Care as a nurse, clinician or administrator.
