Presence Saint Anne Center is a 179-bed skilled nursing facility conveniently located in Rockford, Illinois, designed to provide medical and rehabilitative care for patients who are clinically complex and have multiple acute or chronic conditions. Our patients and staff are supported through our partnership with Nurse Practitioners/Medical Director to optimize the high-quality care we are known for throughout the community.

LPN (part-time, benefit eligible)

Responsible for the provision of nursing care for ministry residents pursuant to resident care plans and physician orders. With aid of registered nurse, monitors nursing assistants and health service aides.


Current license as LPN in the state of Illinois/Indiana in good standing with the Department of Professional Regulations, w orking knowledge of nursing regulations governing long-term care, a ble to work with limited supervision, active c ertification in CPR, a bility to read, write, speak and understand English, a bility to learn and use current technology

Essential Duties and Responsiblities

Collects data and collaborates with the interdisciplinary team in the evaluation of the health status of patient/residents and addresses changes to patient/resident conditions, c ollaborates in the development and implementation of an interim plan of nursing care, p rovides and documents observations and delivery of care according to established guidelines, a ccurately transcribes and implements authorized healthcare professional’s orders, a ccurately administers and documents medications and treatments, a dvocates for patients/patient/residents, p articipates in the evaluation of patient/resident responses to interventions, c ommunicates and collaborates with other health care professionals, p rovides and documents patient/resident/family education, r esponds appropriately to emergencies and protects patient/residents and staff from accident/injury by following safety procedures, m onitors health service aides/nursing assistants to ensure that proper care and services are provided to patient/residents/patients and responds to problems in patient/resident care accordingly , a ssists with orientation and ongoing mentoring of peers
