Related course work is required
NYS LPN License
At least three years experience is required
Must be bilingual (Spanish/English.)
The Licensed Practical Nurse provides nursing care in accordance with the mission, objectives, policies and procedures of the health center and the state Nurse Practice Act. Participates in patient teaching.
Participates in Quality Improvement activities.
Uses an understanding of the principles of growth and development to assist with patient's needs and provide age specific treatment as ordered by provider.
Graduate of an accredited school of nursing, and currently licensed to practice in New York State.
Employees are required obtain BLS certification within one year of the date of hire.
TECHNICAL COMPETENCIES AND PROJECTS
Assigns patients to exam room within fifteen minutes of arrival.
Delivers individualized patient care consistent with established standards of care, policies and procedures, and other regulatory standards.
Demonstrates ability to follow protoc0ls during crisis/emergency situations.
Delivers effective patient care through patient in-take
at the time of appointment.
Insures exam rooms are stocked daily and properly cleaned/organized after each patient.
Maintains the role of patient advocate through support and communication of patients need.
Initiates or participates in continuous quality improvement activities.
Identifies patient's learning needs and provides education utilizing appropriate educational modalities.
Maintains complete, accurate, concise documentation of patient status in the office and via telephone (including date time and signature), nursing interventions and patient responses and outcomes.
Completes all patient call backs (same day, provider, etc..) prior to the end of the work day and complete daily, weekly, monthly assignment check sheets as assigned.
Actively participates in discharge planning needs; coordinates appropriate referrals and schedules tests as required.
Demonstrates proficiency in general nursing skills to provide total patient care in the office based practice.
