Established in 1993, the Cook Family Health Center is a wholly-owned company of the Cook Group and created to provide primary care medical services to Cook Group employees and their families living and working in the greater Bloomington, Indiana area. The Center is currently staffed by 5 full time family practice providers, with approximately 20,000 patient visits and 100,000 prescriptions filled per year.

Primary Function:
Providing patient care in a Family Practice Clinic serving COOK employees and their dependents

Essential Job Functions/ Duties:
• Provide patient care under the direction of Nurse Practitioner or Physician via telephone triage
• Triage incoming calls, relay detailed messages to providers, relay provider messages to patients
• Schedule appointments for procedures and fax orders to service providers
• Scan and index documents to EHR, manage Input module
• Send patient and pharmacy refill requests to physician
• Work as floor nurse/assistant when needed and on assigned Saturday rotation
• Patient Intake
• Assist Provider
• CLIA waived testing
• Phlebotomy
• Scheduling
• Patient Education and Communication
• Administer medication
• IV therapy
• Allergy Immunotherapy
• Nebulizer treatments
• Occupational nursing
• Electrocardiogram
• Sterile technique
• Immunizations
• Computer Skills (EHR)
• Must work and interact effectively with and for others to achieve
company goals
• Must strictly adhere to safety requirements
• Maintain regular and punctual attendance
• Maintain flexible work schedule
• Must maintain company quality and quantity standards
• Must have effective oral and written communications throughout various
levels of the global organization
• Ability to work in collaborative and independent work situations and
environments with minimal supervision
• Ability to remain calm and receptive in fast paced situations
• Compliant with all policies of the company including without limitation the Cook Employee Manual, Cook Code of Conduct, Cook Electronic Information Policy, HIPAA regulations and Cook Policy & Guidance On Interaction with Healthcare Professionals

Minimum Work Experience/ Educational Requirements:
Valid LPN License for Indiana State or eligible for Board exam
Minimum of one year medical office experience

Physical Requirements/ Work Environment
Walking and standing for long periods
Ability to spend significant time on the telephone, sitting for long periods of time
Lift up to 50lbs
Visual acuity for computer work
