esponsible for providing quality, comprehensive nursing care in the home under the direction of a Registered Nurse.
Participates in the development of the plan of care along with other disciplines on the team.

Demonstrates current and up-to-date knowledge of all state and federal governmental regulations and intermediary guidelines.
Demonstrates an understanding and adherence to continuous quality improvement objectives and safety, environmental, and outreach locations to provide care under the direction of the Registered Nurse.
Conducts skilled nursing visits and provides direct patient care as prescribed by the patient’s physician, plan of care, or as directed by the RN and within the Texas scope of practice for Licensed Vocational Nursing.
Regularly travel to patient homes and outreach locations to provide care under the direction of the Registered Nurse.
Accurately documents all patient visits on the day services were performed.
Performs PRN visits and work rotating weekends with other team members.
Receive and draft verbal physician orders as required; obtain co-signature of RN on each order.
Follows up on reported or witnessed patient problems and concerns; maintains excellent communication with physician, client, family members, and RN as necessary.
Prepares proper equipment and materials for treatments using sterile and aseptic techniques as required.
Assists patient with self care techniques; provide training and demonstration of self care techniques.
Maintains compliance with all At Home Healthcare policies, procedures, and regulations, including all federal and state laws.
Makes copies of various documents as required.
Maintains a clean and safe work environment.
Daily review and accurate processing of mail and other correspondence.
Completes accurate and timely documentation.
Accurately prepares and proofreads memos, letters, and other correspondence daily.
Performs all other duties as requested by supervisor.
POSITION REQUIREMENTS, KNOWLEDGE, SKILLS AND ABILITIES:

Requires in-depth understanding of a comprehensive field of knowledge normally attained through a high school education and graduation from an accredited school of nursing. Must have and maintain current license as a Licensed Vocational Nurse in the state of Texas.
Requires minimum of one year prior successful experience as a Licensed Vocational Nurse. Home nursing care experience preferred.
Requires the ability, willingness, and means to daily synchronize laptop computer with network server to ensure proper information exchange and to download software updates. Incumbent may synch from home, public Wi-Fi Hotspot, or work as the last official “act” of the day or
prior to the start
of the official workday
