The Visiting Nurse Association Licensed Vocational Nurse (LVN) provides clinical intervention as assigned to meet patient’s needs; as directed by the Plan of Care. As an integral interdisciplinary team member, our LVN’s provide excellent care and service that is safe, timely, effective, efficient, and patient centered.
Under the direction and supervision of the Registered Nurse Case Manager, our LVN’s provide clinical treatment to home care and hospice patients. This includes the collection of information related to the patient and their immediate health needs. Treatments may include taking vitals, medication and IV administration, wound care, pain and symptom management, patient and caregiver education, addressing patient safety concerns, and overall well being and comfort of the patient.
All LVN duties and patients are assigned and supervised by the Registered Nurse Case Manager. Any changes identified by the LVN should be immediately documented in the electronic health record and communicated to the RN. The LVN will also attend Case Conference to provide input on patients and obtain necessary knowledge to continue quality care.
Graduate of a state accredited school of vocational nursing.
Must be licensed by the State of Texas as a Licensed Vocational Nurse; and in good standing.
VNA Qualifications include:
A minimum of 1 year experience in direct patient care in a healthcare facility or with a home health/hospice agency.
Must demonstrate sound and proven skills in performing clinical procedures, expert level in verbal, written, and electronic health records, and superior time management and organizational skills.
Must possess the characteristics of courtesy, honesty, and integrity to meet the VNA expectations of quality care.
Must successfully pass a background check, satisfactory driving record search, and drug screen.
Must be able to show proof of the following items on the first day of employment:
- LVN license in the state of Texas, in good standing
- Valid Texas Driver’s License
- Automobile Liability Insurance
- A reliable vehicle (driving is required with this position)
- American Heart Association approved CPR Certification
- 2 approved I9 documents indicating eligibility to work in the United States
- Copy of degree/education or transcripts indicating successful completion
- A copy of current TB results or Chest X-ray if available.
