Education and Training:
Graduate from an accredited school of nursing (BSN, ASN, Diploma).
Licensed to practice in this state.
Participates in continuing education so that he/she has knowledge of current trends and developments in nursing and maintains current licensure requirements.
Maintains CPR certification.
Job Knowledge and Capabilities:
Ability to learn, absorb, and apply professional training, follows doctor’s orders and keeps records in a prescribed manner.
Ability to perceive difference in patient behavior and recognize symptoms and the ability to communicate effectively in writing, by telephone and in personal contacts.
Ability to organize work procedures, assume responsibility and tactfully supervise others.
Maintains positive attitude and is an open-minded, supportive member of the Nurse Management team.
Able to be on feet standing or walking 8 – 12 hours per day.
Able to lift 60-70 lbs. on occasional basis.
Able to bend, stoop, squat, and twist numerous times daily.
Able to see and hear adequately in order to respond to auditory and visual requests from patients.
Able to speak in clear concise voice in order to communicate with patients who may be hearing impaired.
Able to carry out fine motor skills and manual dexterity.
Presents a professional appearance at all times.
Specific Duties and Responsibilities Required:
Supervises and evaluates work knowledge and skills of MDS Nurse and Medicare Nurse.
Monitors care provided to be sure policies and procedures are followed.
Assists in recruiting, hiring and orienting new staff.
Organizes and maintains MDS/PPS schedule according to RAI/Medicare guidelines
Completes required assessments, MDS, RAP’s, and Care Plans per schedule.
Signs all MDS as complete in timely fashion.
Reviews and updates CNT Care Cards with Care Conferences.
Carries out disciplinary actions as stated in personnel policies.
Ensures all MDS are transmitted timely.
Schedules and facilitates Care Plan meetings.
Participates in Level of Care meetings.
Participates in the Quality Improvement/Quality Assurance program.
Makes walking rounds.
On-call as scheduled.
Assists DON as needed.
Other duties as assigned.
