DESCRIPTION

The Centerpointe MDS Nurse provides clinical leadership to the neighborhood ensuring the mission vision and values of Lakeview Village by providing clinical leadership and oversight.

The MDS Nurse assures that each client will have the appropriate assessments and evaluations by all disciplines and coordinates their care with other staff in the neighborhood.

DUTIES

  • Develop preliminary and comprehensive assessments of the nursing needs of each client.
  • Develop a written plan of care (preliminary and comprehensive) for each client that identifies the problems/needs of the client, indicates the care to be given, goals to be accomplished, and which professional service is responsible for each element of care.
  • Insofar as practical, encourage the client and/or family to participate in the development and review of his/her plan of care.
  • Ensure that all personnel involved in providing care to the client are aware of the care plan and that care plans are used in administering daily care to the client.
  • Review nurses notes to determine if the care plan is being followed.
  • Review and revise care plans and assessments as necessary follow the state/federal regulations.
  • Develop and maintain a good rapport with all services involved with the care plan to ensure that a team effort is achieved in developing a comprehensive plan of care.
  • Oversee computerization of RAI process, inputting revising, printing and posting as necessary in accordance with Sate and Federal regulations.
  • All other duties as assigned by your supervisor.
QUALIFICATIONS

  • Valid LPN or RN License in the State of Kansas (IV Certified if LPN)
  • 3 months previous MDS experience
