InterCare Community Health Network is a not-for-profit organization, headquartered in Bangor, Michigan with 400 medical and dental health professionals and support personnel, at multiple locations. Provides assessment, social work support services, case management, and psychosocial education to pregnant and postpartum women and to mothers of infants.
Functions as a member of a multidisciplinary team headed by the MIHP Coordinator.
Requires frequent travel and visits into clients' homes.
A Bachelor's or Master's Degree in Social Work required with current Michigan Social Work Licensure.
CPR certification, and one to two years' community health in pediatric, and/or maternal/infant social work services preferred.
