Job Specifics: The Albany Memorial Hospital Manager for Care Coordination is responsible for the planning, priority setting, coordination, implementation and evaluation of programs related to Care Coordination.
Job Duties: The Manager assumes operational responsibility for the overall management of human, fiscal and material resources as well as the integration of the Care Coordination Department into the primary function of the organization. Represents the Operations Division in the planning of multidisciplinary or interdepartmental programs as they relate to care coordination. Participates with other Program and Department Directors in policy development and strategic planning for St. Peter's Health Partners. Demonstrates the organization's commitment to sound ethical business practices as directed by the Corporate Compliance Office and approved by the Board of Trustees.
Minimum Requirements: Graduate/Licensure from a health-related field. Bachelor's degree desired.. Must have in-depth knowledge of healthcare issues. 3 to 5 years of progressive, responsible healthcare management; and strong communication and interpersonal skills with ability to work effectively and collaboratively with nursing colleagues, physicians and other healthcare professionals and administrators.
