OVERALL PURPOSE OF POSITION:
Provides medical social services to restore patients to the highest level of social functioning as possible.
ESSENTIAL: 1. Rehabilitation Center at Wenatchee Valley Hospital
a. Assess social functioning and living situation which existed prior to admission of all patients and evaluate implications for discharge. Identify service gaps that will interfere with patient’s independence.
b. Initiate discharge plan at admission with involvement of patient and family.
c. Compile discharge plan prior to first team meeting and modify as changes occur.
d. Document discharge process in progress notes of medical chart.
e. Provide emotional support for patient and family.
f. Provide information and referral to community agencies based on identified gaps in patient’s ability for self care at home.
g. Facilitate nursing home placement when ordered by physician.
h. Provide counseling when requested by patient or the physician with patient consent.
i. Facilitate weekly team meetings and bring typed list of patients to be reviewed.
j. Schedule and facilitate family conferences.
k. Create and maintain a Quality Assurance program.
l. Facilitate patient and employee support groups as requested.
m. Coordinator for Section 504 of Rehabilitation Act of 1973.
n. Complete and submit IRF PAI patient data to UDSMR organization and provide 90 day post discharge follow-up for UDSMR Quarterly reports.
o. PPS coordination.
p. Ensure paperwork for “ Temporary Pass ” for day and overnight home visits is complete.
q. Create Polar FIM graphic for individual patients and send to referring physician monthly.
r. Print 60% compliance report monthly after all discharges are entered in UDSMR software.

2. Medical Surgical Unit at Wenatchee Valley Hospital
a. Screen new admits for possible social services needs as outline in Social Services Quality indicators policy.
b. Respond to all referrals from physicians, nursing personnel, family and patients.
c. Coordinator for Section 504 of Rehabilitation Act of 1973.
3. Wenatchee Valley Medical Center
a. Respond to all referrals from physicians, nursing, family and patients and complete a patient encounter form (PEF) on the EMR regarding action taken on referral.
b. Organize and facilitate monthly Stroke/TBI Survivor/Family Support Group.
c. Educate staff about the role of the social worker in a medical setting.
d. Compile and update Community Resources Files.
e. Coordinator for Section 504 of Rehabilitation Act of 1973.

1. A Master of Social Work degree from a NASW accredited university/college.
2. Two years previous experience in medical social services desired.
