Job Description

This position will be responsible for supporting the Partners Community Healthcare network in implementing Patient-Centered Medical Home (PCMH). The Partners network has identified this model of care as a strategic imperative, which will allow us to succeed in a new generation of contracts (e.g. AQC, ACO) and lead to an improvement in patient outcomes and an overall reduction in health care costs. This person will work on central activities which support the widespread implementation of Medical Home. Principle Duties and Responsibilities:

Manage data collected from physician practices as they adopt the Virginia Mason Production System.

Manage PCMH team work plan. Coordinate with internal resources who are participating in PCMH activities. Coordinate logistics for Virginia Mason and Stoeckle Center training sessions. Coordinate practice site visit schedules and special events related to PCMH implementation.

Track PCMH cost center expenses.

*
Stay informed about national and regional developments related to Medical Home. Gain knowledge of the Virginia Mason Production System, Lean, and other key components of Medical Home implementation.

Develop effective relationships with community physicians, physician leadership, and PCPM/Partners staff.

*
Educate physicians, administrators, staff and other key stakeholders about the components of Medical Home and the benefits and challenges of transitioning to that model of care.

*
Support Medical Home Consultants in consulting and coaching practices as they implement Medical Home. Address external and internal questions and inquiries as directed by the Medical Home Consultants.

Support Medical Home Specialists on activities related to evaluating the impact of Medical Home and achieving NCQA recognition.

*
Assist the Medical Home Specialist in the development and publication of Medical Home marketing material and reports tracking progress.

*
Capture user/prospect feedback on ways to improve our Medical Home program.

*
Assist team in building consistency with other Partners sites in the rollout of Medical Home.

Other duties as assigned

Qualifications

BS/BA or commensurate experience required

*
Excellent organizational skills

*
Good communication skills

*
Understanding of the health care environment

*
Strong project management skills

*
Ability to work independently

*
Position requires local travel to physician practices
