Responsible for the growth, development and management of hospice care services provided in over 50 contracted facilities (i.e. Nursing Homes, Skilled Nursing Facilities (SNF), Assisted Living Facilities, Hospitals etc.) across regions. Advances the clinical expertise of hospice staff, which deliver care in the skilled nursing facilities and inpatient settings. Coordinates with the Hospice Liaisons to facilitate and ensure a seamless referral/admission process for facility patients.

Educates and supports facility staff regarding the VNSNY hospice care model. Ensures the implementation of clinical/quality improvement initiatives for facility hospice services in compliance with regulatory requirements. Works under general direction. Licensure: License and current registration to practice as a Registered Professional Nurse or Social Worker in NYS preferredEducation: Degree in Nursing, Health Care Administration, Community Health, Social Work or other related field required.

Masters degree in same disciplines preferred.Experience: Minimum of six years experience working in a Hospice program, including two years supervisory/management experience required. Strong public/vendor-relations skills, marketing and presentation skills required to promote VNS's Hospice program and enhance outside facilities awareness of VNS required. Strong communication skills, both oral and written, with demonstrated ability to provide counseling, and deliver training and/or education programs required. Working knowledge of personal computers including Microsoft Office (Word, Excel, Outlook) and the ability to generate reports from the SunCoast system required.

Demonstrated direct and indirect leadership skills required.
