QUALIFICATIONS: Required, Bachelors Degree. Current RN license in Illinois. Desired, Previous experience in case management. Knowledge of workmans comp insurance.

ESSENTIAL FUNCTIONS: Initiates clinical contact with patients of a defined risk profile and facilitates primary and specialty care of chronic and acute illness. Manages the functional operations of IOCP with support from internal and external program leads. Works with primary care providers to conduct pre-visit planning and post-visit follow up. Assists and motivates patients to enact behavior/ life-style changes that positively impact their health and welfare. Develops and implements individualized education plans for patients and families utilizing suitable instructional materials and methods. Identifies and refers to appropriate social services; Acts as patient advocate with agencies for participating patients, as needed.Coordinates outreach visits to assist patients to keep clinic appointments; includingescorts or transport as needed. Participates in clinical treatment planning as a member of the treatment team, IOCP care management. Documents timely, accurate and appropriate clinical information in patients¿ medical record. Monitors individual treatment plans in conjunction with the providers to ensure efficient and timely care. Maintain a good working relationship with hospital departments, physician offices and specialty testing sites to expedite patient appointment scheduling and timely results reporting. Facilitates patient and family engagement in carrying out physician instructions and following treatment plans. Assist in clinical areas providing patient care as needed. Maintain confidentiality of all patient information. Perform other duties as assigned. Additional Functions, Clinical, Demonstrates clinical competence. Implements appropriate actions to respond to needs of patients and their significant others. Monitors and documents the patient¿s response to therapeutic interventions and progress toward outcomes. Demonstrates patient first and cost effective approach to patient care. I. Provides a milieu conducive to communication and cooperation among patients, families, provider and staff. Identifies and communicates key issues and trends to managers and makes recommendations for improvement on an ongoing basis. Establishes relationships across departments to optimize patient care. IV. Education, Assesses and communicates staff development needs to manager. Serves as a resource for staff. Contributes to unit orientation. V. Research, Assures that research activities are completed and recorded according to protocols. Participates in nursing research activities. AA/EEO.
