DESCRIPTION

Program Description: Cecil Ivory House is a transitional housing program targeting formerly homeless clients who are either mentally ill or dually diagnosed as mentally ill and chemically addicted. Funded and licensed by the NYS Office of Mental Health with additional funding from the US Department of Housing and Urban Development, Cecil Ivory House is located in Harlem and has a capacity of 24 beds. The mission of the program is to provide a safe and supportive environment where residents take part in rehabilitation interventions that will assist them in the attainment of their work, social and community living goals. A variety of services are available, including case management, substance abuse groups, medication monitoring, daily skills training and recreational activities.

DUTIES

Develop policies, protocols and forms regarding medications and health management. Administer medication. Make referrals for medical treatment; and follow up with psychiatric rehabilitation programs, treating physicians, pharmacies, and other treatment collaterals. Serve as the primary medical case manager for assigned clients. Provide individual and group therapy; medical referrals. Facilitate in-service trainings for staff and residents. Attend case conferences as needed. Manage client health needs. Run health-related educational groups. Basic health assessment including blood pressure check, weight, etc. and referrals as appropriate. Participation in treatment team meetings. Assist in organizing and participating in goal development and related activities. Weekly chart documentation and interaction with medical and psychiatric providers. Maintain medication administration record and controlled substance log. Monitor and count medication and medication supply. Related duties as assigned.

Fulltime: 37.5 hours a week
Monday-Friday 1pm-9:30pm

QUALIFICATIONS

LPN license required. Experience in psychiatric and medical nursing preferred. Excellent organization skills. Basic computer literacy required.
