Exempt ; full-time; 40-hours per week; varied days & hours; full-benefits

This professional position conducts in-depth assessments and screenings of prospective residents from the acute care, community and assisted living settings, including psychosocial, physical, medical and environmental and financial parameters. This position will perform on-site assessments to determine appropriateness of admission and work with prospective residents and their families on the front end of the admission process to ensure a seamless transition into the Citizens Care and Rehabilitation Center (CCRC) and Montevue Assisted Living community, relying on experience and judgment to plan and accomplish goals. Supervision is received from the Executive Administrator .

Examples of Work:
  • Work with the Director of Nursing, or designee to ensure that all admissions are screened thoroughly and adequately prior to admission
  • Apply review criteria to assess and document the appropriateness of each admissions, including length of stay and level of care
  • Complete nursing admission assessment for each resident, to ensure appropriate patient placement; oversee bed assignments and completion of other preliminary paperwork for entering residents
  • Develop costing of medications, equipment and potential outpatient treatments
  • Act as liaison between the facilities and family members during the pre-admission and admission process; after admissions if needed, refer family members to the appropriate Administrator or Director depending on specific need
  • Closely monitor number of facility residents and Medicare census; work aggressively to appropriately place potential residents to maximize reimbursement
  • Conduct tours and open houses; provide facility-specific information and event information on specialized units and other resources
  • Work with the Social Services staff to ensure that all advanced directives have been discussed prior to admission, so that appropriate paperwork can be in place upon admission
  • Obtain admitting physician orders, and narcotic prescriptions prior to residents admission to facilitate transition into the facility
  • Ensure that all admission documents are completed in a timely manner and that all forms are placed in the medical record
  • Develop, track, trend and report an accurate census of admittees, including inquiries, referral sources, and length of stay
  • Collaborate with staff to design and develop marketing strategies, including fliers brochures, factsheets and newsletters to promote business opportunities for CCRC and Montevue; recommend and implement initiatives that manage the target audience’s perception of CCRC and Montevue
  • Work with business office staff to confirm that an adequate payment source is in place prior to each admissions (i.e. Medicare, Medicaid, Managed care, private insurance or personal funds)
  • Serve as primary representative of CCRC and Montevue in the local community, building relationships that result in referrals while continuing to be available at CCRC and Montevue to consult with potential residents and their families
  • Provide monthly and annual reports to the Quality Assurance Committee, the Executive Administrator and the Montevue Assisted Living Manager
  • Coordinate special events for the facilities and the community; as needed, assist other staff with the promotion and coordination of any special events
  • Offer general information on nursing home care, assisted living and related topics, such as advance directives, estate planning, understanding the Medicare and Medicaid process, and other health-related issues such as Alzheimer’s disease
  • Collaborate with the Respiratory Care Unit to determine placement of ventilator-dependent residents
  • Perform other related duties as required

Qualifications & Requirements:
  • Minimum 2 years work experience in a long term care facility
  • Current license as a Registered Nurse* or Licensed Practical Nurse issued by the MD State Board of Nurse Examiners (licensure must be maintained throughout term of employment)
  • Knowledge of Medicare and Medicaid billing practices
  • Ability to demonstrate considerable initiative and judgment in determining psychosocial and psychophysical needs of potential residents
  • Discretion and good judgment in dealing with confidential matters, sensitive information and the financial/personal information of others
  • Ability to effectively access and utilize computerized systems including word processing, spreadsheet and data management
  • Ability to effectively make independent decisions, organize work, determine priorities and complete assigned duties with minimal supervision, as the majority of work is performed outside of the facility
  • Strong and effective spoken and written (English) communication skills, including public speaking skills and the ability to effectively present information at trainings/meetings
  • Physical requirements include the ability to move/carry medical equipment and to lift, move and assist residents as needed
  • This position and the physical workplace may include exposure to air contaminants, hazardous chemicals, infectious waste, diseases and medical conditions, etc. (e.g. Tuberculosis, AIDS virus, Hepatitis B virus, etc.)
PREFERENCE MAY BE GIVEN FOR :
  • Bachelor's degree in Nursing
  • Long term care admissions experience
  • Marketing work experience

Additional Information/Examination Process:
  • Ability to provide own transportation as needed
  • Available for varied working days and hours, which may include weekdays, weekends, holidays and on-call for emergencies
  • This position is subject to random drug testing
  • The Citizens Care and Rehabilitation Center and Montevue Assisted Living are tobacco free facilities; use of tobacco products is not allowed on the campus
  • * If the candidate hired into this position possesses RN licensure, the candidate will be hired at a higher salary of $52,032 per year.
