The Nurse Assessment Coordinator is responsible for the Minimum Data Set (MDS) process for the Transitional Care Unit of Sierra Nevada Memorial Hospital. MDS is a process mandated by the Center for Medical and MediCal Services for all patients in Medical or MediCal certified medical facilities. This process is a carefully documented clinical assessment of patients, and set electronically to the Center for Medical and MediCal Services. Responsibilities include organizing the delivery of care for patients including administering procedures for care plans, completing chart reviews, quality assurance and continued reviews on patients to receive maximum Medicare or MediCal reimbursement.
