Nurse Care Manager Min Salary $43,611.00 Reporting to the Care Management Unit (CMU) Supervisor, the RN Care Manager provides community-focused case management services to enrolled members within the context of an Inter-Disciplinary Team (IDT) to adults who are elderly, physically disabled, developmentally disabled, and/or with mental health or substance use disorders, based on a social work model of practice. The RN completes comprehensive health assessments on members, skillfully assessing their psychosocial, medical, and other conditions as related to identifying the individuals' attributes, goals, and challenges. Along with the Care Manager and member, the RN plans and coordinates long-term care services which are person-centered, of high quality, and cost-effective. The RN also assesses the need for primary, secondary, and terciary levels of health care and provides members with needed referrals, education, and coordination of treatment. The RN Care Manager identifies clinical and functional supports that are needed to address the barriers to the member's outcomes and identifies risks, developing plans to mitigate and reduce such risk. The RN Care Manager coordinates health care services for Family Care members, including home health services, acute and primary care, and nursing home and residential care. This employee also makes needed referrals to multiple service providers and links members to resources, evaluates the effectiveness of services and member satisfaction with services, and acts as an advocate for the member in terms of their rights to privacy, self-determination, and health care. Caseloads average approximately 70 members per RN. Locations will include the counties of Chippewa, Dunn, Eau Claire, Pierce and/or St. Croix. Qualifications include: *Current licensure in Wisconsin as a Registered Nurse. *At least one year of experience working with one or more of Family Care's target population (frail elders, developmentally/physically disabled adults). *Valid Wisconsin Driver's License, sound driving record, and unlimited access to transportation. *Intermediate computer skills including Microsoft Office Suite (Word, Excel) and familiarity with web-based programs.
