NURSE NAVIGATOR

Department: College of Health & Human Services

Salary: $56,657

Job Summary
Under general administrative direction, coordinates referrals from physicians and health care facilities for patients; provides a significant level of patient education related to their illness and planned treatment.

Provides cancer care education and emotional support for patients with cancer diagnoses in a holistic manner and in partnership with other healthcare professionals.

Coordinates patient triage with community collaborators.

Assists in training Community Health Workers.

Provides Gail Risk Score calculations to staff, obtains patient appointments and provides feedback of mammograms and clinical reports.

Duties
Provides cancer care education and emotional support for patients with cancer diagnoses in a holistic manner and in partnership with other healthcare professionals.

Coordinates patient triage with community collaborators.

Assists in training Community Health Workers.

Provides Gail Risk Score calculations to staff, obtains patient appointments and provides feedback of mammograms and clinical reports.

Communicates with all members of the healthcare team on behalf of the patient.

Provides support and counseling, as appropriate to the clinical situation.

Communicates with referring physician's offices as appropriate to the patient and physician's needs.

Works with marketing and outreach departments to educate referring physicians and facilities on available services.

Performs various duties as needed to successfully fulfill the function of the position.

Knowledge, Skills and Abilities
- Knowledge of contemporary clinical nursing practices and procedures related to cancer diagnosis and treatment
- Ability to organize and interpret information relating to oncology policies, procedures, and programs
- Skilled in oral and written communication

Qualifications
Graduation from an accredited school of nursing. Bachelor's of Nursing Degree required. Possession of a current, unencumbered RN license from the Texas Board of Nursing.
