Meridian Health Services has been serving the community for over 35 years and is accredited by the Joint Commission.Meridian Health Services is currently seeking a Nurse Practitioner . This position provides an array of nursing care services and related functions as a member of a multi-disciplinary treatment team. Actively collaborates with staff psychiatrists in assessing and monitoring clients' physical and mental health status. Working hours are Monday through Friday from 8am to 5pm with one (1) required weekend per quarter.

DUTIES

Provides direct nursing care using advanced scientific knowledge, nursing theory and skills to: identify abnormal conditions, diagnose health problems, develop and implement treatment plans, and evaluate patient outcomes.Assesses, collects and performs patient/client physical examinations using a systematic approach, demonstrating knowledge of the principals of growth and development across the lifespan.Makes differential diagnoses of symptoms, functional problems, and risk behaviors, including non-disease-based etiologies. Integrates the unique needs of patient/client and family by developing interventions to enhance intended and to prevent unintended consequences. Partners with patient and significant others to develop a health plan. Evaluates therapeutic interventions for effectiveness, efficiency, cost-effectiveness, consumer satisfaction, and ethical considerations.Constructs aggregates of patient/client characteristics that facilitate development of population-based programs of care.Develops treatment plans and coordinates plans across inpatient/outpatient services with modifications required to deliver care that is population specific.Provides indirect nursing care through planning, guiding, and evaluating care delivered by multidisciplinary staff.Facilitates to nursing practice that require change, enhancement, or maintenance.Conducts/applies nursing research as appropriate to the area of practice. Assists staff in critiquing and appropriately applying nursing and related research to nursing practice.Identifies researchable patient/client problems or needs for clinical inquiry projects and process improvement.Uses advanced knowledge and skills in teaching ad guiding nursing personnel.Uses and designs appropriate tools to identify gaps in the knowledge, skills, and competencies of nursing personnel.Develops and implements effective staff development programs that target the needs of staff to further patient care and nursing practice. Mentors nursing staff in acquiring new skills and in career development. Creates an environment that stimulates self-learning and reflective practice. Identifies significant organizational relationships that are facilitators or barriers to any proposed change across the continuum and at points of service.Analyzes organizational differences within and between groups that can affect implementation of change. Identifies factors (e.g., culture, finances, regulatory requirements, external demands, and other variables) that influence outcomes. Collaborates with professionals from multiple and varied disciplines to integrate interventions to enhance patient care.Leads nursing and multidisciplinary groups in implementing patient care programs that address patient care issues across the full continuum of care, different population groups, and/or different specialties. Collaborates with patients/clients and other healthcare professionals, as appropriate, to monitor progress and make modifications as needed. Contributes to the development of interdisciplinary standards of practice and guidelines for care (e.g., pathways, care maps, benchmarks, guidelines). Teaches and counsels individuals or groups by utilizing communication skills and teaching or learning theories to increase knowledge or functioning of health care team members.Participates actively in creating a quality educational environment and delivering educational services that are flexible and responsive to identified needs of all customers: patients, staff, and community.Addresses population-specific interest in session development. Participates in periodic evaluation of services rendered, including, but not limited to, the following: chart reviews, case reviews, patient evaluations, and outcomes. Demonstrates skill in monitoring and evaluating the effects of solution strategies on patient/caregiver outcomes, on nurse and other healthcare providers, and on cost-effectiveness.Selects methods of evaluation that best fit the grogram of care and desired outcomes.Documents the results of evaluation efforts in a reportable fashion. Serves as a professional role model.Assists in orientation of new personnel as assigned. Serves as a resource person in area of expertise. Attends staff/committee meetings as requested.Attends national meetings as appropriate. Seeks national certification, if applicable. Provides Patient, Staff, and Customer SafetyCreates a work environment that promotes safety for patients, staff, and customers. Reports any safety concern or near miss on the appropriate report form. Maintains awareness of safety at all times.

QUALIFICATIONS

Graduate of an accredited school of nursing with a master's degree in nursing from a Nurse Practitioner program.Must be eligible for, or have perscriptive authority under the criteria outlined by the Indiana State Board of Nursing.Current license as a registered nurse in the state of Indiana, or eligible for Indiana licensure.Psychiatric Nurse Practitioner certification and two (2) years' experience preferred.Experience with Psychiatric consults, med-surge & ER consults beneficial.
