United Hospice of the Midlands - Aiken, part of the UHS-Pruitt family of companies, has an immediate opportunity for a Nurse Practitioner.Job Description: The Nurse Practitioner provides Medical Care to all residents as assigned by the Staff Physician and/or Medical Director. Performs general physical examinations on eligible residents and employees to assess their health status, assess existing health problems implement preventative measures. Managers minor acute and chronic illnesses for residents. Conducts and interprets with physicians? laboratory outcomes, diagnostic studies and procedures. Prescribes, administers and evaluates pharmacotheraputic and non pharamacotheraputic regimens. Consults and collaborates with physicians and other health care professionals to manage complex health problems. Develops outcome measures to monitor quality of care and effectiveness of services. Conducts and utilizes research that contributes to the improvement of patients/family outcome. Refers veterans requiring further evaluations to appropriate clinics. Offers education and counseling to patients and families concerning results, meaning of examinations, tests and on preventative medicine strategies particularly in regard to promoting health and well being. Identifies individuals, families and communities at risk. Assists patients to maximize their abilities to manage states of health and illness. Serves as a mentor and consultant for nurses, physicians and other healthcare professionals. Facilitates access to VA health care and ensures the appropriate utilization of resources. Utilizes resources in an efficient and economic manner to ensure that each resident achieves the highest practicable physical, mental and psychosocial well-being. Complete all other tasks as required by the Medical Staff Physician, Medical Director or above.Licensure/Education Requirements: Must be a graduate of an accredited school of nursing. Must have current, active license as a Nurse Practitioner.Experience Requirements: One year professional experience in the nursing field in a long term care setting.
