Evercare is now part of the OptumHealth division under the greater UnitedHealth Group. As an OptumHealth Nurse Practitioner (NP) or Physician Assistant (PA) you will provide primary care to your caseload of patients in long term care settings. This excellent opportunity affords a flexible autonomous role bringing enormous satisfaction in the care and comfort of our aging population. Additionally, you will build meaningful relationships with the residents and their families.

Responsibilities for Nurse Practitioners / Physician Assistants include:
-Providing direct care to aging residents in long-term care facilities
-Collaborating with primary care physicians, facility staff and patients' families
-Participating in care conferences with other providers and patient family members
-Working with utilization review process for each facility
-Identifying patients' behavioral health issues
-Educating nursing home staff, physicians and administration
-Coordination of care for patients

Requirements Positions in this function require license and certification as Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist. Provides primary case management of chronic and acute conditions.
