The BHSI ICM unit is seeking to expand capacity and revolutionize under the guide of the DBH/MRS Addiction Services and CBH to provide services to all Philadelphia County Residents who are uninsured, underinsured, and those that are health choices eligible. The Office of Addiction Services Intensive Case Manager (OAS ICM) Fee for Service will coordinate formal treatment and provide support to individuals that have extensively utilized treatment and social service agencies, chronically homeless and including but not limited to individuals that may suffer with psychiatric disorders. The OAS ICM will support individuals seeking self sufficiency and recovery for substance abuse disorders through providing on going support and ancillary services to individuals who are Health Choices eligible; funded by Community Behavioral Health (CBH) to increase the retention in and completion of treatment of recovering people in order to support their efforts toward recovery and self-sufficiency; and, to increase recovering people's access to core services such as primary health care, psychiatric care, stable and secure living environment, positive support networks, vocational training, and employment. The OAS ICM should be available after hours for on call emergencies via cell phone.

Duties and Responsibilities:

Maintain a maximum caseload of 25 crisis cases.

Obtain 100% of the available billable units based on the available hours per month.

Maintain a compliance documentation rating of 95% each month following the documentation rating matrix.

Monitor the quality of treatment through social services agencies, residential and outpatient treatment facilities.

Participate in the individual's treatment experience by closely working with the recovering person's care manager, therapist, psychiatrist, social worker, etc.

Integrate treatment plans with the Case Manager's Service Plans.

Contribute and participate in all after care planning to provide continuity of care.

Monthly supervision incorporates clinical support and a thorough review of all Case Managers' active charts, productivity review and discussion of ICM needs and/or challenges.

Attend Quarterly supervision with Assistant Director of Case Management Consult regularly with CBH'S Clinical Management Team.

Coordinate clinical interventions as needed.

Attend all mandatory trainings as designated.

Skills required: Must have good verbal/ written communication skills and work well with people; Critical decision making skills, ability to work independently, and be flexible/ adaptive to a changing environment. Knowledge of MS Word and Excel.
