Purpose of this position:
The purpose of this position is to evaluate & treat individuals with impaired ability due to disease, injury, or degeneration, utilizing the skills and expertise of a licensed Occupational Therapist and within the OT scope of practice.
The primary functions and responsibilities of this position are as follows:

Current Indiana Ocuupational Therapist Assistant licensure.
Must be organized, self-directed and highly motivated.
Must be able to speak, read, write and understand English.
Patience, tact, enthusiasm, and positive attitude toward the elderly.
Must be able to lift 50 pounds occasionally. Finger, hand dexterity, visual and aurel acuity is necessary. Position requires sitting, standing, stooping, walking, bending, pushing, pulling, stretching, lifting, grasping, and reaching.
Delegation of authority: Authority is delegated to the individual in this position to:
Follow physician orders and facility policies and procedures to treat patients and complete required documentation of treatment provided under the direction and supervision of a licensed OT.
Provide effective direct patient treatment in accordance with established plan of care and physician orders
Participate and/or lead a daily huddle.
Accurately take measurements of patient's status, within the limits of scope of practice and ability
Accurately document skilled intervention provided and patient performance according to established goals and plan of care
Maintain communication and collaboration with the evaluating therapist including any decline in patient's functional or medical status, adverse reactions, lack of significant progress, readiness for discharge, etc. in order to promote maximum benefits of treatment plan
Implement appropriate preventative measures, home exercise or restorative nursing programs, adaptive equipment, etc. to sustain or prevent decline in patient's functional status
Implement education plan to meet the needs of the patient, family, or caregiver
Effectively communicate/collaborate with patient, family, caregivers and other members of the healthcare team to promote maximum benefit of care
Complete all orders under the direction of the evaluating therapist, timely and in accordance with facility policies and procedures
Complete all clinical documentation completely, accurately, legibly and timely in accordance with regulatory requirements
Assist therapist in goal reassessment and discharge planning
Follow plan for discharge and discuss with evaluating therapist any additional concerns as they arise
Effectively complete patient screens
Demonstrate appropriate knowledge of clinical techniques, procedures and programs, use of available modalities and equipment for clinical setting and patient population served
Effectively use discretionary time to perform tasks beneficial to the rehabilitation dept
Meet established productivity standards
Appropriately manage physical resources of the dept by demonstrating fiscal awareness and respectful and proper use of equipment
Exhibit professionalism through demeanor, appearance, and communication
Demonstrate consistent knowledge and application of Medicare and other reimbursement regulations
Seek professional growth and development through continuing education, mentoring opportunities, learning and applying new skills, etc.
Shares expertise and support with other team members
Other functions or tasks:
Prefer at least 2-5 years of clinical experience in a Long Term Care and Rehabilitation Facility. New Grads are welcome to apply.
