POSITION TITLE : Occupational Therapist
POSITON STATUS : 35 hours per week, 3 days a week
QUALIFICATIONS: D egree in occupational therapy with current N.Y.S.
license and registration. Two years of experience working
with the developmentally disabled in a clinic or day program
Oversees the delivery of OT services of all individuals requiring OT interventions. Supervise COTA’s in regards to the clinical practices. Directly responsible to the Director of Day Services and Assistant Director of Day Services Responsible to the Supervising OTR for clinical direction.
Provides assessments of participant’s strengths and needs and makes recommendations with regard to neuromuscular, musculoskeletal, postural, and cardiopulmonary and developmental disabilities and habilitative potential. Develops and implements individualized program plans based on evaluations in accordance with personal goals and preferences. Provide documentation for Day Hab programs as follows: Initial and follow up evaluation and consultation Obtain physician order for OT treatment and splint wearing Daily clinical service record documenting the date, response code and consumer response to treatment Annual and Semi annual reports and evaluations as needed Acts as a member of the interdisciplinary team, and makes suggestions with Regard to the program plan for each participant. Attend and prepare for semi annual, annual, and any other meetings necessary. Communicate with residential OT to ensure integration of all OT programs between the residential program and Day program Provides consultation and instruction to direct care staff with regard to occupational therapy interventions, taking into consideration the consumer’s goals and preferences. Provides consultation and training to agency personnel including, but not limited adaptive equipment, dining, sensory stimulation, and improving abilities for independent functioning. Act as a member of the Dining Team providing dining assessments as needed with the speech language pathologist and the dietician. Conducts assessment of individual’s assets and liabilities with regard to fine motor skills, range of motion, strength, muscle tone, endurance, coordination, perceptual and sensorimotor skills and activities of daily living. Conducts assessment of consumer’s need and use of orthotic and prosthetic devices, assistive and adaptive equipment, positioning, etc. as these relate to occupational therapy as well as drafting medical justifications. Develops an Individual Goal Plan or Ongoing Service Plan for each service recipient indicating goals, objectives and treatment methodology as appropriate. Provide transfers of program participants to and from wheelchairs, therapy tables, toilets and other adaptive equipment Recommends purchases or develops material for delivery of OT services. Performs all other duties as directed by the Director of Day Services or Assistant Director of Day Services
