JOB SUMMARY:

Carries out established therapy treatment programs according to the policies and procedures of the Regional Therapy Center. Demonstrates an understanding of human development in order to effectively evaluate and treat patients of all ages for diagnoses the individual is competent to evaluate and treat.

RESPONSIBILITIES:

Establishes therapeutic rapport with patients, residents, families and care givers (Refer to Bornheim Scale #4).

Consults with other members of the interdisciplinary care team to coordinate therapeutic programs and to coordinate the discharge of the patient or resident from therapy in collaboration with the responsible therapist. (Refer to Bornheim Scale #6)

Assists and collaborates with the responsible therapist in order to identify and prioritize problem areas to be addressed during therapy. (Refer to Bornheim Scale #6)

Performs treatments according to the established plan, utilizing appropriate procedures, equipment and techniques within the scope of the profession and according to the policies and procedures of the Regional Therapy Center. (Refer to Bornheim Scale #1)

Monitors the patient's or resident's response to treatment and modalities as indicated in collaboration with the responsible therapist. (Refer to Bornheim Scale #2)

Reports responses orally or in writing to the responsible therapist. (Refer to Bornheim Scale #7)

Prepares written reports for the medical record according to established guidelines on evaluation results, progress in and reaction to therapy, and discharge from therapy. (Refer to Bornheim Scale #7)

Educates patient/resident and/or caregivers about established goals and methods of treatment. (Refer to Bornheim Scale #7)

Fits and adjusts patient supportive and/or assistive devices and/or braces in order to carry out established treatment plan. (Refer to Bornheim Scale #1)

Assists with departmental maintenance to ensure availability of clinical supplies and forms. (Refer to Bornheim Scale #10)

Demonstrates a personal routine for job performance which facilitates increased productivity. (Refer to Bornheim Scale #3 & 9; and average daily productivity expectations.)

Facilitates third party payment for services by requesting pre-authorization for therapy visits, submits supporting documentation as required. Also submits required statistical & operational information on a daily basis. (Refer to Bornheim Scale #2)

Leads or directs the activities of attendants, volunteers, aides and students, ensuring that policies, procedures, and applicable laws are enforced, and that quality patient care is provided. (Refer to Bornheim Scale #5)

Participates in the quality assurance and continuous quality improvement program as assigned. (Refer to Bornheim Scale #8) QUALIFICATIONS:

Certification (or eligible) to practice in New York State as an occupational therapy assistant. Must also be certified by National Board for Certification in Occupational Therapy as a COTA. Hospital based or skilled nursing facility experience preferred.
