Processes requests for PCW/HHA assignments Tracks patients’ progress with the vendor agencies Maintains PCW/HHA personnel records and keeps Business Manager informed regarding missing documentation and PCW/HHA duty sheets. Inputs patient’s PCW/HHA schedule and changes in the schedule into the Comprehensive Care Management computer system. Handles billing problems as they arise: re: incomplete/incorrect information or investigation - other reimbursement alternatives. Processes requests for DME and supplies for CCM patients and maintains proper documentation. Filing as needed. Performs general clerical duties and related functions as required. Coverage for receptionist and other business personnel/medical practice staff as required. Other duties as assigned.
