The Oncology nurse navigator will provide individualized assistance to patients, families, and caregivers to help overcome healthcare system constraints and facilitate timely access to quality medical and psychosocial care from screening and pre diagnosis of cancer through all phases of the continuum of care into survivorship. The Navigator works closely with physicians in all specialty areas to coordinate the patient’s plan of care and communicates with all members of the team on behalf of the patient.

Key Responsibility Areas:
  • Facilitate the entry of patients into the cancer program and coordinate care.
  • The Oncology Nurse Navigator is the point of reference for referring physicians seeking evaluation of patients with suspicion of Cancer.
  • Follow the patient throughout the continuum of cancer care services from referral through treatment and support care to survivorship.
  • Evaluate progress of patients along the continuum.
  • Work closely with the Physicians in all specialty areas to coordinate patient’s care plan and work with the multidisciplinary team to maintain and implement an up to date coordinated care plan.
  • Develop a relationship with all Cancer Program physicians and serve as a liaison between specialists and patient’s referring physician.
  • Provide patient education across the continuum of care and provide symptom management and test results for patients.
  • Provide access to community resources and links patients to support services.
  • Assist clinic staff, research team, and physicians with accruing cancer patients to clinical trials.
  • Assist with tracking of metric and quality indicators for diagnosing and treating cancers promptly and effectively.
  • Work with cancer survivors and other health care team members to determine a survivorship program and develop long term plan for patients.
  • Collaborates with the healthcare team in data collection and statistical activities to facilitate problem identification and resolution to improve quality of patient care.
  • Prepare for and coordinate Multidisciplinary Conference.
  • Provide patient and physician support for procedures.
  • Assume other leadership roles as required by the departments

**Depending on Practice Site Requirements, an incumbent in this position may not perform all the key responsibilities listed below. However, it is expected that at least 80% of the responsibilities are performed for this job. Travel to other sites may be required.

Education and/or Experience Required:

Current license as a Registered Nurse in Illinois

Required: Bachelor’s degree in Nursing

Preferred: Master’s Degree in Nursing

Preferred: Certification in area of expertise

3 years experience in Oncology Nursing

Thorough knowledge of the healthcare system , community, and social service resources for cancer required.

Computer Proficiency: Able to learn and become proficient in EPIC as well as have basic knowledge of Microsoft office applications, ie. Outlook, Word , Excel; and demonstrate keyboarding speed and accuracy at a minimum of 30 WPM.
