Physician Assistant or Nurse Practitioner Licensed in the State of Texas. The Orthopedic Coordinator is a member of the orthopedic care team that provides patient centered, evidence based and holistic care for all orthopedic patients. OC practices and in a manner consistent with the mission and goals of the organization.
Rounds on orthopedic patients daily to ensure appropriate plan of care, progress and outcomes. Documents rounds in interdisciplinary progress notes.
Collaborates with nursing staff and members of the clinical team to ensure appropriate plan of care for each patient that includes discharge instructions, medication reconciliation, prescriptions and other appropriate information is provided to the patient before discharge.
Consults with orthopedic surgeons and attending physicians and accompanies them on rounds daily when applicable.
Reviews physician documentation to assure all required documentation is present and current i.e. H&P, signatures time and date, progress notes.
Reviews documentation for compliance with recommended Medicare Administrative Contractor (M.A.C.) rules and regulations.
Encourages evidence based practice through the development and implementation of order Sets, protocols algorithms in collaboration with orthopedic surgeons, orthopedic team members and order set committee.
Meets daily with orthopedic team members to ensure the plan of care for each patient is implemented, current and addresses patient care needs.
Ensures that all core measure components are addressed with appropriate documentation present in the patient's medical record.
Chairs the orthopedic team in pursuit of Orthopedic Surgery/Joint Replacement Program Certification by TJC.
Provides appropriate patient care orders as condition warrants within scope of practice.
Completes discharge summary at discharge and or in time frame noted in the Medical Staff By-Laws.
Provides orthopedic education to nursing staff and serves as a patient care resource.
Educates patients and their families during hospitalization. Documents education in Interdisciplinary Education Record. Conducts preoperative orthopedic surgery class for total joint and back surgery patients.
Physician Assistant or Nurse Practitioner Licensed in the State of Texas.
Clinical Orthopedic experience. Masters Degree required.
