PROGRAM/UNIT: Homeless Outreach
HOURS: Per Diem

DUTIES/RESPONSIBILITIES: Responsible for providing direct outreach, escort, and referral services for all homeless individuals encountered in the field during shift. Provide case-management to individuals in BRC Programs. Assist individuals with obtaining benefits and complete psycho-socials and housing applications. Also responsible for recording and maintaining accurate statistical data on all individuals encountered and referred as well as case-record management of all clients receiving longer-term services and treatment plans. Provide emergency first aid/CPR assistance when needed. Related duties as assigned. QUALIFICATIONS: Preferred: B.A in Social Work or several years of equivalent Para-professional experience in direct services to the homeless mentally-ill and substance abusing population. Basic computer literacy required. Valid driver’s license required. CPR training certification or willingness to take training class in CPR. Alternate: High School Diploma or GED. Willing to train highly motivated individuals who are committed and sensitive to the needs of the street dwelling population. Valid driver's license required. CPR training certification or willingness to take training class in CPR.
