The Outreach Worker will work within a team setting to provide assertive street outreach, clinical engagement and linkage services for individuals who are struggling with homelessness and mental illness. This team will focus outreach activity on the north and downtown areas of Chicago. The worker will enter all relevant clinical documentation on a daily basis in the Member’s electronic medical record, participate in team meetings and provide for recovery based services. The worker will use agency-owned vehicles to perform street outreach and follow-up visits in the community. The worker will participate in regularly scheduled supervision sessions.
BA/BS Degree or MA/MS Degree . Experience in providing assertive outreach services to unsheltered individuals a plus, promotion of recovery oriented services and strong advocacy skills.
This person should have good written and verbal communication skills and strong basic computer skills, which include the ability to document services in a clear and effective manner. Experience in the mental health field is strongly preferred.
Must have a valid Driver's license and good driving record.
Must have 100,000/300,000/100,000 auto liability insurance.
