The PCMH Strategist will provide subject matter expertise related to all aspects of PCMH, recognition programs and overall practice transformation to support the AAFP Patient-Centered Medical Home (PCMH) Initiative. The PCMH Strategist will develop and support tools and resources that can be used in a variety of channels (web content, articles, and editorials) necessary to effectively transform to a PCMH.

The PCMH Strategist will participate in various internal committees and external groups focused on quality and PCMH including the Patient Centered Primary Care Collaborative (PCPCC) as well as providing support to the Commission on Quality and Practice. 5-10 days out-of-town travel annually.

Required Skills

Required Experience

Bachelor’s degree in a related field or equivalent work experience plus at least six years of directly related work experience required. (Master’s preferred).

Required skills and experience: Strong knowledge of PCMH concepts and PCMH recognition programs; project management; ability to effectively research literature and synthesize large amounts of information into meaningful tools or processes; ability to write concise content that can be: 1) adapted for the web, publications, tools or resources and 2) used as reports to key stakeholders; and effective communication skills essential to collaborate effectively with internal and external stakeholders required.

Preferred skills and experience: Knowledge of patient-centered health care practices, practice redesign, change management, performance measurement, and performance improvement; creation and presentation of PowerPoint presentations; and knowledge of practice redesign.
