DESCRIPTION

Be a member of a Primary Service Provider Team conducting individual evaluations, on-going assessments and provide educational support to parents/caregivers identifying developmental learning opportunities in their child's natural enviroment. This includes attending regular PSP Team meetings as appropriate.

DUTIES

Duties include but are not limited to:
1.)Participate as a member of a multidisciplinary evaluation team and complete the Battelle Developmental Inventory-2 or other appropriate developmental evaluation tools to determine eligibility for the Early Steps Program, following the eligibility criteria established in the Early Steps Program Handbook and Operations Guide. (PHOG)

2.) Be an active member of the Individual Family Support Plan (IFSP) Team for children deemed eligible for the Early Steps Program. Make appropriate recommendations for specific functional outcomes and early intervention services.

3.)4.) Maintain requirements of IDEA Part C and HIPAA.

4.) Maintain a minimum monthy productivity rate of 74%.

5.) This position requires travel. Candidate must have their own transportation and proof of current vehicle registration/insurance. Travel will be remibursed.

6.) Perform other duties as assigned.

QUALIFICATIONS

Qualified candidates must possess:
  • A minimum one year hands on professional experience with children birth to age five with special needs and their families.
  • Credentials as required for enrollment as an Early Steps Provider in the Children's Medical Services Provider Management System
  • Florida Professional licensure as a Occupational Therapist, Physical Therapist or Speech/Language Pathologist
  • Florida Medicaid Therapy Provider number
  • National Provider Identification Number
  • Valid Florida driver's license
  • Current Florida automobile insurance
