Position Summary

Under the general direction of the Family Practice Physician/Medical Director at the Sandia Health Center, provides oversight of the daily management of the Community Health Program. Provides a full range of public health nursing services to the community of Sandia Pueblo. Services are provided in patients’ homes, the health clinic, senior meal site, and community center and are provided to all age groups from prenatal through senior citizens.

Supervision Exercised

Supervised by the Medical Director in coordination with the Health Center Director. Supervises the CHR Safety Officer, medical CHR, and Medical Transport driver.

Major Duties and Responsibilities

  • Plans, conducts and evaluates a comprehensive community health nursing program for the Pueblo of Sandia community. .
  • Assesses and diagnoses needs , and sets up a plan of action to meet these needs by providing health services to families and individuals.
  • Provides primary care for acute and chronic health problems in collaboration with medical clinic staff, and homecare program.
  • Directs all activities of medical and safety CHRs and medical transport driver, including organization and documentation of their activities.
  • Teaches recognition of early symptoms of communicable diseases and the importance of immunizations.
  • Provides in-home healthcare to all populations from pre-natal care to geriatric care. .
  • Plans and participates in Programs for Diabetes Management and Primary Prevention.
  • Plans and participates in Programs for Communicable Disease Control.
  • Provides skilled nursing services to individuals, families, including measures to promote wellness, to prevent complications and to minimize disabilities.
  • Conducts epidemiological and community surveys, field investigations and research studies to appraise health needs.
  • Participates in continuing quality improvement activities of the Public Health Nursing program.
  • Establishes and staffs immunization/vaccination clinics as appropriate.
  • Maintains records and reporting systems according to Sandia Health Center policies for patients.
  • Works with the Tribe, State and Federal Health Agencies, in planning health related activities.
  • Performs other duties as assigned.


    Minimum Qualifications, Education and Experience

    1. Current RN Licensure with the NM Board of Nursing required.
    2. Bachelors Degree (BSN) in Nursing required.
    3. Three (3) to five (5) years experience in public health nursing required.
    4. Two (2) to three (3) years supervisory experience required.
    5. Must have phone access at all times.

    Licensing Status

    1. Current RN Licensure with the NM Board of Nursing required.
    2. Must submit a recent report of Tuberculosis (TB) skin test upon employment and provide a TB skin test annually.
    3. Must have a current CPR first aid Card.
    4. Must be able to successfully pass a Background and Character Investigation in compliance with PL 101.630.
