POSITION SUMMARY (Briefly describe the position)

The PAP Clerk is responsible for preparing and verifying label against prescription and medicine for strength, dosage and type of medicine for distribution of medication and pharmaceutical supplies in accordance with facility standards and procedures and regulatory requirements. Work is performed under the direct supervision of an MD, Nurse Practitioner and/or pharmacist.

REQUIRED SKILLS AND QUALIFICATIONS

Education :

• High School

Experience : (Required) Experience

• Bilingual English/Spanish

• Proficient Microsoft Office and Excel (familiarity with medical terminology).

Licensure/Certification :

• Employees are responsible for maintaining individual certifications as required by job function or by law and provide

verification and recertification when requested by management.

Duties and Responsibilities:

Personal-Team-Organization

• Inputs needed medication to Rx Assist or equivalent system to identify assistance program available

• Verifies that patient has qualifying social security number or income, refers patient over age 65 to Benefits Counselor

• Researches, locates and downloads patient application

• Communicates with patient and reviews patient chart to identify potential allergies, reactions etc.

• Enrolls patient in program to receive free medications from pharmaceutical manufacturer/distributor

• Documents patient demographics in Rx+ and gives free 30 day supply of medication to patient pending arrival

of 3 month supply from Patient Assistance Program

• Receives requested 3 month supply of medications for patient and logs into Rx+ or equivalent system

• Communicates with patient and reviews patient chart to identify potential allergies, reactions etc.

• Assists MD/NP/pharmacist in providing pharmaceutical care to include preparation of medication, screening phone

calls/orders and alerting the MD/NP/pharmacist to patient-specific data indicating potential medication reactions.

• Receives and stocks medication to the patient care areas via automated medication distribution or non-automated

systems and is responsible for maintaining these systems.

• Assists the MD/NP/pharmacist to deliver medication to MD/NP/Pharmacist for patient with acute condition requiring

immediate and/or emergency medication assistance.

• Maintains confidentiality and protects sensitive data at all times.

• Adheres to organizational and department specific safety standards and guidelines.

• Demonstrates exceptional customer service and interacts effectively with physicians, patients, residents, visitors,

staff and the broader health care community.

• Works collaboratively and supports efforts of team members.

• May perform specialty referrals as required subject to standards outlined in Referral Coordinator job description.

• Maintains a clean and safe environment for co-workers which includes preparing work area for next shift and using

effective communication with oncoming shift.

• Demonstrates a commitment to service, organization values and professionalism through appropriate conduct and

demeanor at all times.

• Assists MD/NP/pharmacist in providing pharmaceutical care to include preparation of medication, screening phone

calls/orders and alerting the MD/NP/pharmacist to patient-specific data indicating potential medication reactions.

• Prepares prescription label form Dispensing Solutions or equivalent system ready for application and

distribution by MD/NP/Pharmacist to patient

• Performs regular, timely dispensary/pharmacy inspections to identify expired medications, submit medication reorder to designated management party for approval, prepare purchase order and submit to vendor.

• Maintains perpetual inventory in system and performs physical inventory as required by Finance
