Candidate verifies all physician orders for patient care, negotiation of prices and the frequency, level and duration of care to be reimbursed. Assurance of customer satisfaction by providing one point of initial contact for clients and referral sources, identifying and anticipating their needs, finding ways to meet them, and verifying satisfaction with two weeks of the start of care.

The Patient Care Coordinator performs duties in accordance of and under the direction/supervision as defined by the Agency's organizational chart.


A BSN is preferred. A background that includes a position in home care management preferred.

Candidates must have at least one year in acute care nursing and at least one year of home care experience.

This position may be an RN or LPN with applicable degree/credentials and currently licensed in the state. Must have a current CPR card.
