Carries primary responsibility for evaluation and care of all patients referred to Hospice of Palm Beach County for hospice services, within the designated facility. Assists referring physicians and-or designee in the coordination of the plan of care-treatment for all HPBC patients within the facility. Serves as the primary liaison between HPBC and the designated facility, as well as the physicians and other healthcare professionals working within that hospital-long term care facilities and other agencies which interact with the institution. Carries shared responsibility for marketing tasks with the Hospice Representative within the designated facility.

Florida licensed Registered Nurse with minimum of 2 years medical surgical experience. B.S.N. preferred. Must possess ability to assess physical and psychosocial needs of patient and family. Demonstrates excellent verbal and written communication skills. Organized, flexible, self motivated with ability to relate to all levels of the professional health care team. Ability to develop and apply effective customer service skills. Able to work independently with minimum supervision. Bilingual (English/Spanish) preferred. EMR experience a plus.
