Pediatrice Palliative Care: You will have a huge impact on the care and treatment of symptoms in a very important sector of healthcare. This individual provides support and education to the familiies of patients with a wide range of needs. Often suffering from Birth anomalies, defects, life altering circumstances. The Palliative care approach is designed to ease the suffering and pain of a small population within healthcare sector. Will work with patients in their home or at long term care facilities . Monday through Friday 8a to 5p. Must have strong pediatric clinical skills, and the desire to work closely with the interdisciplinary team to ensure patient comfort and family support.
Compensation & Benefits Overview
Each RN will be given a lap-top computer, pager and cell phone with which to communicate with the office and patient's families. All may be used for personal use. The cell comes with 2500 minutes per month, should a nurse go over 2500 mins, they are responsible for any overages.
They offer direct deposit with their credit union, as well as starting in May/June short and long-term disability.
Seasons Hospice recognizes eight paid Holidays per calendar year for regular full time employees (40 hours a week) who have successfully completed their 90-day orientation period.

If more than one holiday falls in an employee's 90-day orientation period, the first holiday will not be paid, but subsequent holidays will be paid. These holidays are exclusive of the vacation/sick time accrual.
A Paid Time Off Bank accrues for each full time employee (40 hours) after successful completion of their 90-day orientation period. Accrual equals 20 days from the date of hire to the anniversary of the hire date 12 months later (for 40 hours a week). Accrual is prorated for 32 hours a week. Less than 32 hours does not accrue paid time off. All vacation, sick, personal, pre-arranged paid time off comes from this bank. Exceptions are jury duty and 4 days of bereavement for 40 hours a week full time employees.
Required Minimum Time Off: 50% of the accrued time must be used each anniversary year or it is lost.
Carry Over: No more than a maximum of 80 hours of accrued time may be carried over to the next year up to a maximum of 80 hours days. The employee handbook will explain this in more detail.
MENTAL HEALTH DAYS:
Four per calendar year. One Per Quarter. These days do not accrue; the time must be used in the quarter or lost. This applies to regular full time employees only and is separate from vacation/sick time. Please see the handbook for complete details and restrictions.
Health Insurance is available in two forms from Blue Cross/Blue Shield of Illinois. There is a PPO PROGRAM (Preferred Provider) and an HMO program (Health Maintenance Organization). Please see the brochures for details on each program.
Employee Only Coverage = $12.64 Per Pay Period Payroll Deduction
Single +1 coverage $41.87 per pay period, and Family is $71.10 per pay period.
PPO Coverage59.20 Per Pay Period Payroll Deduction, Single +1 = $131.52
and $207.00 for Family coverage per pay period, payroll deduction.
Each program also includes a Prescription Drug benefit that pays for prescriptions after a co-pay by the employee/insured.
TERM LIFE INSURANCE:
Term life insurance and accidental death & dismemberment insurance is available to Seasons Hospice full time regular employees for 1 times their annual salary (Up to a $50,000 maximum). Cost is $2.00 per pay period for this benefit.
A Dental program is made available for employees who wish to enroll. Employees are responsible for 100% of the cost for dental insurance.

· · Benefits include 100% coverage of preventative care, $1,000 annual maximum per covered individual, $50.00 annual deductible, 80% coverage of basics and 50% coverage of major work.
ELIGIBILITY FOR INSURANCE:
Employees are eligible on the first of the month 30 days after date of hire. Enrollment must be made in the first 30 days of employment. (Minimum 32 hours per week.)
There is no guarantee of coverage. The insurance company reserves the right to decline coverage after review of an application for coverage.
Seasons Hospice also offers a 401k plan to all employees 21 or older after one year of employment .
