The Personal Care Assistant provides culturally sensitive and respectful personal care and home management support for service recipients in their chosen place of residence.

1. Reports to: Services Coordinator, or as otherwise designated.

2. Other: Represents the agency in community activities, trainings and visibilities with stakeholders as assigned.

Essential Functions
Provide quality care in a respectful, sensitive, supportive, and timely manner. Review and follow the Individualized Service Plan developed by the Services Coordinator or designee. Provide service recipients with in-home care/home maintenance, meal planning/preparation correspondence and errands, and other tasks as agreed upon by service recipient and Services Coordinato as documented in the Individualized Service Plan. Assist service recipients to maintain a clear, safe, and comfortable environment, and perform duties indicated by the service recipient's individualized Service Plan. Report any changes in service recipient status or condition immediately to the Services Coordinator or Manager, as appropriate. Accurately complete and submit in a timely manner all required reports and documentation, to include but not limited to: service delivery records, time sheets, mileage records, and incident reports. Maintain service recipient and family member confidentiality in accordance with agency standards. Attend agency in-service and educational trainings/meetings, as required, and other meetings as deemed appropriate by the Coordinator. Maintain good relationships with stakeholders (including current and potential clients, community and co-workers), focusing on providing excellent and culturally sensitive customer service and presenting positive image of the agency in all community and stakeholder interactions. Maintain a working environment which encourages mutual respect, promotes respectful and congenial relationships between employees and is free from all forms of harassment. Perform other duties as assigned, including presentations to community groups and organizations, in accordance with the agency's "team" philosophy. QUALIFICATIONS:

To perform this job satisfactorily, an individual must be able to perform each essential performance requirement satisfactorily. The requirements listed below are representative of the training essential functions listed above.
Education/Training and Experience
Documented evidence of status on the Texas Department of Health nurse aide registry with no findings of service recipient abuse or neglect, or misappropriation of service recipient property, required. At least one year's experience in direct care of service recipients in an institutional setting, required. At least one year of full-time experience within the last five years in direct patient care in a home health or institutional setting, required. High School diploma or GED, required. Must have current certification as a Nurse Aide with the Texas Department of Health, required. Bilingual (English/Spanish) desirable. REQUIRED CERTIFICATES
Certificate or license, as applicable, by the appropriate State agency as a qualified practitioner. State of Texas vehicle operator's license and proof of liability insurance. PHYSICAL DEMANDS

The agency's hours of operation are extended and non-traditional, and may vary based on office location and/or stakeholder requirements. Employees are asked to work the schedule that is best suited to the achievement of deliverables and stakeholder requirements based on the agency's posted business hours, or as required by stakeholder and contractual requirements. Employees who work in an exempt position are paid on a salaried basis and are not entitled to overtime pay. The amount of the salary is intended to compensate the employee for all hours worked to perform the required job.
