Status: Part-TimeLocation: Kokomo Hourly wage: $8.25

Responsibilities: Provide companionship for the client including but not limited to: talking and listening, reading aloud, providing social and emotional support. Promote the client's mental alertness through involvement in activities of interest which promotes a sense of well being. Provide general attention to clients' non-medical needs in accordance with Service Plan. Provide a clean, safe and healthy environment for clients and family members. Provide light housekeeping tasks including but not limited to: washing dishes, laundry, and vacuuming. May prepare, cook and serve meals as directed. This may include assisting with meal planning and grocery shopping. Assist clients in personal tasks such as making phone calls, letter writing, walks, beauty salons, banking, Doctor's office, and community outings as needed. Reminders ONLY for clients to take their medications. Assist the client with activities of daily living by providing approved personal care activities including but not limited to: bathing, dressing, toileting. Observe and report any changes in the client's mental, physical, or emotional condition or home situation to supervisor. Document client activities in daily notes. Perform duties as assigned in the clients personal care Service Plan. Route all receipts to the Coordinator of Personal Care. Participate in IDT (Inter-Disciplinary Team) meetings, as requested. Participate in staff meetings. Must be capable of meeting the physical demands required by caring for the clients. Physical requirements may include, but not be limited to; bending; reaching; stretching; lifting 25-50 pounds (depending on person served) without assistance; grasping; visual acuity; running; walking; squatting, guiding; transporting; assisting with physical transfer; and manual dexterity to handle medications; and handling physical aggression as defined in CPI training. Must be capable of working scheduled shifts. Must be able to accept direction from supervisor and other members of management. Must be able to function independently; follow written instruction; and assess physical and behavioral situations in order to respond appropriately to the needs of the client. Must provide personal hygiene assistance to the persons served.

Minimum qualifications: •As per State regulatory requirements, must be at least 18 years or older.
•Attained a minimum of a High School Diploma or GED.
•Must be capable of obtaining and maintaining certification in CPR, First Aid,
•Posses a valid Indiana Driver's License and driving record that allows insurability as determined by the agency's liability insurance company.
•Maintain a clear criminal record, especially with regard to neglect or abuse.
•Must satisfactorily complete agency orientation and departmental training program.
