- 1-3 years of experience
Performs all pharmacy distribution (medication order entry, preparation and dispensing of medication), in accordance with pharmacy policy and procedure and all state and federal laws.
Performs clinical pharmacy duties (pharmacokinetic consultations, DUE/MUE studies, drug information, ADR reporting, Code Blue coverage).
Medication order entry: accurate computer entry with evaluation for appropriateness of dose, route, interactions and cautions with consideration of age specific criteria, co-morbidity and other clinical parameters.
Intervenes, consults, and recommends as required and utilizes the Pharmacist Intervention Database, when necessary.
Understands Investigational Drug Protocols and proper control.
Minimum 1 Year (2 to 3 years preferred) of Inpatient Acute Care Hospital Pharmacy experience.
Ability to work independently under the direction of pharmacy management.
Ability to follow all hospital and pharmacy policies and procedures.
Ability to interact with other pharmacists, technicians, interns, other pharmacy staff, nurses, physicians, other hospital staff, patients and patient’s families. Demonstrated ability to supervise all pharmacy personnel.
Pharm.D. or Bachelor of Science in Pharmacy.
Registered Pharmacist – active California Board of Pharmacy licensure required. Working knowledge of Pyxis C-II safe and Pyxis 3000.
