BASIC FUNCTION: To perform assessments and develop care planning; responsible for implementation, communication, and coordination of services with other health care providers and community agencies. To monitor and evaluate the options and services required to promote a patient’s health needs with quality, cost effective service. To provide physical therapy treatment directed toward preventing disability, relieving pain, developing, improving and/or restoring muscle function and maintaining maximum performance within the client's capabilities in accordance with the Plan of Care.

RESPONSIBILITIES/TASKS: NOTE: Some descriptions may have more specific duties and/or goals and objective attached to this form. Such attachments normally reflect unique aspects of specific locations, shifts, departments, etc.

*1. Supervise, monitor, evaluate and provide direction to direct reports.

*2. Performs a complete evaluation of client’s home environment; makes recommendations for changes in accordance with the client’s needs.

*3. Performs specialized therapy skills and professional assessments; directs patient care as prescribed by the patient’s physician, plan of care, and within the State of Texas’ scope of practice for Physical Therapist.

*4. Provides safe and accurate physical therapy services to the client in the client home as prescribed by the physician.

*5. Performs evaluation of client at each visit; reviews records and reports; immediately notifies physician of any change in client’s condition, response to therapy, and relevant recommendations for changes in patient’s plan of care.

*6. Provides instruction and supervision of direct reports in physical therapy services provided to client.

*7. Provides training and demonstration to client and family members on physical therapy uses and devices applicable to the client’s total physical therapy program.

*8. Monitors and evaluates services performed by Physical Therapy Assistant; ensures client care is rendered safely and in compliance with plan of care prescribed by the physician.

*9. Demonstrates current knowledge and adherence of state and federal regulations and intermediary guidelines for home health care.

*10. Demonstrates thorough understanding and adherence to continuous quality improvement objectives, safety, environmental, and infection control standards.

*11. Effectively manages an assigned caseload of patients; effectively maintains current knowledge of assigned patient’s conditions, needs, and responses to treatment necessary to provide highest quality of care to client regularly.

*12. Regularly travels to patient homes and outreach locations.

*13. Provides independent care to clients in remote locations.

*14. Participates in or communicates to case conferences with interdisciplinary team members to evaluate and discuss patient diagnoses, treatment plans, progress, goals, scheduling, problems, and interventions as required. Accurately and timely completes all documentation relevant to patient in patient file.

*15. Assist health care providers and physicians in developing and implementing care plans; works with patients and family members to ensure effectiveness of care plan prescribed by physician.

*16. Accurately, completely and timely document all physical therapy visits and assessment on the day services were performed and on appropriate agency clinical forms.

*17. Responsible for implementation of care plans; makes recommendations to physician for necessary revisions to care plan as required.

*18 . Responsible for accurately administering manual therapeutic exercises; provide instruction, motivate, and assist clients in non-manual exercises and functional activities. Properly administer traction, massage, and application of physical agents treatments to clients as prescribed by the physician in the plan of care.

*19. Evaluate, fit, and adjust prosthetic and orthotic devices; recommend changes to the physician as necessary.

20. Performs all other duties as requested by supervisor.

POSITION REQUIREMENTS, KNOWLEDGE, SKILLS AND ABILITIES: NOTE: These requirements represent minimum levels in order to perform the job on a satisfactory basis. Candidates must have the ability to satisfactorily perform the essential functions of the job.

1. Requires a pleasant and cheerful demeanor, shows an attitude of helpfulness and

cooperation, and the ability to encourage, mentor, and support fellow workers on a daily

basis and while encountering stressful situations. Will be required to effectively and efficiently carry out the duties of this position.

2. Requires in-depth understanding of a comprehensive field of knowledge attained through graduation from an accredited physical therapy curriculum, approved by the American Physical Therapy Association of the Committee on Allied Health Education and Accreditation of the American Medical Association or the Council on Medical Education of the American Medical Association and the American Physical Therapy Association.

3. Must have and maintain current valid license as a Physical Therapist in the State of Texas.

4. Minimum of two years prior successful physical therapy experience required; prefer physical therapy experience in home health care environment.

5. Requires ability to speak, read and write English.

6. Requires ability to react effectively to emergency situations.

7. Must have and maintain valid Texas Driver’s License and proof of current liability insurance.

8. Must maintain current certificate in basic life support (CPR).

9. Must submit to annual TB screening.

10. Requires ability to use up to 100 pounds of force occasionally and/or up to 50 pounds of force frequently and/or up to 20 pounds continuously to move objects.
