Palm Beach Habilitation Center is a private non profit agency that provides training for over 600 adults with disabilities to develop the skills to live and work to their highest level of independence.
DUTIES
Provide case management and program planning for assigned case load. Develop and implement an Implementation Plan for each assigned individual. Serve as liaison to Family members, Guardians and other interested professionals in order to attain comprehensive services.
QUALIFICATIONS
Must be at least 18 years of age and qualify for Level II background screen. Bachelor Degree, prefer degree in Human Service Field and prefer experience in case management and or in direct service or program planning for individuals with disabilities. Valid driver license required and clean driving record. Provide case management and program planning for assigned case load. Develop and implement an Implementation Plan for each assigned individual. Serve as liaison to Family members, Guardians and other interested professionals in order to attain comprehensive services.
