Classification: Grade 11 (Local 580) FLSA Status: Non-Exempt Position Description: Professional; A Public Health Nurse is a professional registered nurse certified by the State of Minnesota who specializes in population based Public Health nursing practice. The practice is system, community and individually focused to protect entire populations from disease, illness, and injury and promotes physical and mental health. To accomplish this mission, public health nurse services balance three core government public health functions. Those core public health functions are: · Assessment determining what needs to be done by assessing the community health status and available resources · Policy Development being part of the solution through proposals to support and encourage better health · Assurance making sure that it happens through necessary high quality, effective services available in the community The 6 Areas of Public Health Responsibility are as follows: · Assure an Adequate Local Public Health Infrastructure · Promote Healthy Communities and Healthy Behavior · Prevent the Spread of Infectious Disease · Protect Against Environmental Health Hazards · Prepare For and Respond To Disasters and Assist Communities in Recovery · Assure the Quality and Accessibility of Health Services
Position assignments may include but is not limited to: Maternal Child Health home visiting, WIC (women, infant child), Case Management for home and community based services, personal care assessments, immunizations, jail health, disease prevention and control, preparedness, and health education, etc.

Minimum Qualifications (Pass/Fail): Public Health Nurse (PHN) Certification in Minnesota ( must attach Certification ); one (1) year related work experience; valid driver’s license and reliable means of transportation; successfully pass a pre-placement screen, successful completion of a Child Protection Background Check (M.S. 299C.60, as amended), and Sexual Exploitation Background Check (M.S. 604.20, as amended), all contingent upon a job offer. Knowledge, Skills & Abilities: Ability to work well with others; ability and willingness to follow rules and procedures; ability and willingness to follow directives from supervisors. Preference for: PHN work experience.
WORK HOURS, LOCATION, CONDITIONS AND EXTENT OF SUPERVISION :
Work Hours: This posting is for a Regular, F/T position, Monday - Friday, between the hours of 8:00 a.m. - 4:30 p.m., overtime as necessary and approved. Work Location: Itasca County Health and Human Services Department, Itasca Resource Center, 1209 SE 2nd Ave, Grand Rapids, MN
