1. Performance Improvement
Coordinates the development and implementation of quality projects/programs/initiatives in the office setting.
a. Understands Vidant Health Physicians safety and quality program framework and communicates that throughout organization.
Provides expertise in assessing, evaluating and monitoring improvement strategies at clinic level. Develops and refines, in collaboration with clinical and medical leadership, action plan for quality improvement . Utilizes the model for improvement (PDCA) to facilitate improvement. Assists Quality Administrator in evidence based research dissemination.
b. Maintains close familiarity with patient workflows, documentation, and clinical issues across Vidant Health Physicians.
Coordinates staff level quality meetings. Provides direct assistance to the Vidant Health Quality Administrator to assure that organizational issues are addressed and reported through Vidant Health Physicians Safety/Quality Committee and/or appropriate Vidant Health Leadership. Provides education and guidance to clinic QI Committees and activities to adequately apply all available legal protections of quality data, discussions and activities. (Consults with Legal as needed)
c. Coordinates Clinic Quality Improvement Activities
Coordinates collection, validation and submission of quality data (including but not limited to clinical quality, CLIA, JCAHO, etc). Participates in root cause analysis addressing sentinel events, as well as data and trending analysis of sentinel, critical and event data. Communicates resource and budgetary needs to the Vidant Health Quality Administrator Networks with leadership to assure exemplary, comprehensive, cost-effective care delivery.
2. Practice Evaluation
Monitors quality action plans to achieve targeted outcomes. Responsible for leading performance improvement at physician practices.
a. Collaborates with Vidant Health Quality committee, Quality Administrator, and VP Medical Affairs to support timelines and achieve target outcomes in a timely and effective manner.
Establishes broad focused outcomes with attention to patient care, patient and family collaborative care initiatives, population health/disease management activities, and staff satisfaction. Leads clinic level incorporation of evidence based practice to support population health strategies.
b. Obtains and analyzes information from research, as well as internal and external benchmarks to evaluate outcomes effectively. Assists Quality Administrator in utilizing SPC and CQI tools that are most appropriate to measure, analyze and display data. Ensures data is accurate, timely and understandable. Networks with IS to automate and streamline processes, and to optimize the use of electronic systems and efficient use of data sources.
c. Coordinates the monitoring, evaluating, and improving of quality performance of Vidant Health Physicians clinics (includes clinical quality, CLIA, safety and infection control rounds)
Ensures the QI program is in compliance with regulatory standards and evidence base practice criteria. Participates in benchmarking projects, reviewing charts and completing data collection forms, reviewing final data for implications for change in practice. Facilitates multidisciplinary review through webex or other media, QI meetings, and supports peer review processes. Monitors the suggested corrective actions and ensures the loop is closed.
3. Communicates effectively with all Vidant Health Physicians staff and physicians to achieve quality outcomes. Serves as resource/consultant.
a. Functions as effective change agent to achieve desired quality outcomes and to ensure progress of quality initiatives.
Develops productive relationships with office staff, physicians, clinical staff, and operations managers throughout Vidant Health. Networks with a variety of disciplines and service areas to improve communications, enhance understanding and to strengthen relationships. Shares information among quality partners to expand best practices and promote change.
b. Maintains excellent written and verbal communications with quality partners. Mentors clinic staff to be active participants in the quality and change processes.
c. Serves as quality and safety consultant across Vidant Health Physicians through committees, task forces, departmental meetings, staff meetings, and other forums. Serves as a consultant regarding quality program planning and development, including the use of SPC and CQI tools and processes. Consults with quality peers throughout the region, health system, and state.
4. Ensures compliance with accrediting agencies. Incorporates evidenced based research and best practice criteria a. Monitors compliance with regulatory standards, licensing bodies, and other accrediting standards. Stays abreast of changes in standards/criteria and communicates same throughout Vidant Health Physicians.
b. Works with departments/services/ priorities to become compliant as issues arise. Assists as requested in clinic site visits and prepares summary documentation.
c. Incorporates current research findings into quality initiatives. Is aware of and provides evidenced based references as needed to support QI initiatives.
5. Contributes to clinic education advancement
a. Provides planned, systemic and just-in-time education on QI related topics, clinical documentation through EMR, and other vital topics.
b. Participates in educational conferences through health system, EAHEC and/or NCAHQ.
c. Uses teaching/learning theories and principles to foster the educational development of others. Adjusts presentation style and materials to meet the needs/experience of the audience.
Minimum Position Requirements
BS degree in Nursing required.
Minimal 3 years experience in clinical leadership, quality outcome monitoring, or disease management/population health required.
5 years clinical experience required.
Current NC RN licensure required.
