This position exists to process Home Care referrals for home nursing, home infusion, home therapies, home medical equipment and supplies. The incumbent is a liaison between Home Care Services and physician, clinics, hospital personnel, third party payers and other community agencies. The incumbent serves as a resource to external and internal customers. This position is responsible for insurance verification, eligibility, and pre- certification.
