Instruction and supervision to individuals participating in the Residential Habilitation Program. The Residential Habilitation Instructor works with individuals in their own homes to increase their independence in activities of daily living. Responsible for ensuring that confidentiality is strictly maintained concerning all agency related information.

•Provides direct instruction to Residential Habilitation participants to increase their independence in the areas of daily living, including self care/personal hygiene, food skills, community integration and leisure recreation activities.
•Communicates medical concerns to Service Coordinator
•Assists in developing Residential Habilitation Plan with individual/treatment team
•Maintains frequent contact with Program Manager and Program Administrator and communicates as needed with Service Coordinators/Community Service Providers and family/guardians
•Participates in general staff meetings, in-service training and other pertinent agency meetings
•Assumes responsibility for Residential Habilitation program and assists individuals during assigned hours which may include days, evenings and weekends
•Safely transports individuals to and from activities and appointments
•Completes daily log, Quality Assurance Checklists, consumer files and other paperwork as necessary
• Performs CPR and Strategies for Crisis Intervention and Prevention (SCIP-R)
•Complies with all agency policies and procedures
•Other duties as assigned
•High School Diploma or GED
•18 years of age
•NYS Drivers License that meets agency policy
•Ability to read and write
•Ability to handle multiple tasks simultaneously
•Ability to meet physical requirements of the position when performing the following: standing, walking, sitting, bending, squatting, kneeling and climbing
•Lifting requirement of 50 lbs
•Physical agility and ability to react to emergency situations, including maintaining Strategies for Crisis Intervention and Prevention Core Training certification (SCIP) as per regulatory guidelines
•Ability to lift and transfer individuals manually and mechanically as required by prescribed program /plan
•Ability to move, reposition and place individuals in pieces of equipment
