Are you looking for an opportunity to use your RN skills in a fast paced and exciting environment? We are looking for highly skilled RN's to join our team as RN Case Managers in Pierce County, WA (Tacoma)!

This is a Monday through Friday position. Our Case Managers are telecommuters, but approximately one-half of their time is spent visiting patients in their homes. As an RN Case Manager, you will be responsible for the management of care for our Medicare and Medicaid members. Members are generally 65+, but there are also members under 65 that are medically complex with chronic illness.

Primary responsibilities include:
Comprehensive care management.

Care coordination and health promotion.

Comprehensive transitional care from inpatient to other settings, including appropriate follow-up.

Individual and family support, which includes authorized representatives.

Referral to community and social support services, if relevant.

The use of health information technology to link services, as feasible and appropriate.

Requirements

Required Qualifications :
Valid RN license with no restrictions in state where the position is located

Minimum 3 years adult or geriatric clinical experience

Computer knowledge required (Internet, MSWord, Excel, Outlook)

Passion and commitment to excellence

Positive attitude and solution oriented

Ability to handle multiple tasks

Ability to work well as part of an interdisciplinary team

Willingness to travel throughout Pierce County as needed.

Preferred Qualifications:
Possess an understanding of care management

Knowledge of disease processes for chronic illnesses in the senior population

Experience in managed care, disease and/or case management

CCM or CCP certification or eligibility

Knowledge of evidenced based medical guidelines
