The Visiting Nurse Association Registered Nurse is responsible for the assessment, planning, intervention, and ongoing evaluation of patients in home care and/or hospice. As an integral interdisciplinary team member, our RN’s provide excellent care and service that is safe, timely, effective, efficient, and patient centered. As a Case Manager, the RN ensures that all members of the care team follow the physician’s plan of care and communicates necessary changes to the physician and the interdisciplinary team.
The RN Case Manager is responsible for the delegation and supervision of LVN’s and C.N.A’s. The RN assists with training and the onboarding process for new clinicians and acts as a resource/mentor to others on the health care team.
Our RN’s provide clinical treatment to home care and/or hospice patients. This includes the assessment and collection of information related to the patient and their immediate health needs. For home health a strong knowledge of OASIS and 485’s is strongly preferred, but not required. The RN must have expert level clinical assessment skills and be able to make independent decisions for the well being of the patient. Treatments may include taking vitals, medication and IV administration, wound care, pain and symptom management, patient and caregiver education, addressing patient safety concerns, and overall well being and comfort of the patient.
The RN Case Manager utilizes the electronic health record in the home to capture and communicate patient information. The Case Manager attends Case Conferences to give and receive information to improve patient care.
Registered Nurse with Associate Degree or Diploma in nursing. Bachelor’s degree preferred.
Must be licensed by the State of Texas as a Registered Nurse; and in good standing.
VNA Qualifications include:
A minimum of 1 year experience in direct patient care in a healthcare facility or with a home health/hospice agency. Home Health experience preferred.
Must possess the ability to make independent nursing judgments with minimal supervision.
Must demonstrate sound and proven skills in performing clinical procedures, expert level in verbal, written, and electronic health records, and superior time management and organizational skills.
Must possess the characteristics of courtesy, honesty, and integrity to meet the VNA expectations of quality care.
Must successfully pass a background check, satisfactory driving record search, and drug screen.
Must be able to show proof of the following items on the first day of employment:
RN license in the state of Texas, in good standing
Valid Texas Driver’s License
Automobile Liability Insurance
A reliable vehicle (driving is required with this position)
American Heart Association approved CPR Certification
2 approved I9 documents indicating eligibility to work in the United States
Copy of degree/education or transcripts indicating successful completion
A copy of current TB results or Chest X-ray if available.
