To provide quality, comprehensive nursing care in the Patient’s home as prescribed by the physician in the plan of care.
Responsible for collaborating with the interdisciplinary team to develop the plan of care for the Patient. Manages nursing treatments and activities to meet the needs of each individual patient.

Demonstrate current and up-to-date knowledge of all state and federal governmental regulations and intermediary guidelines.
Demonstrates proficient understanding and adherence to continuous quality improvement objectives and safety, environmental or infection control standards.
Performs specialized nursing skills including professional assessments and direct patient as prescribed by the Patient’ physician, Plan of Care and within the scope of practice for professional nursing in the state of Texas.
Responsible for evaluating utilization of patient services and patient progress; review evaluation with the care team, patient, family members, physician, and case manager as needed.
Required to attend case conference and participate in discussion of patient progress.
Responsible for initiating patient discharge planning upon patient admission to care. Accurately and timely complete Discharge Summary at time of patient discharge from agency services.
Performs HHA supervisory patient visits as required.
Participates in quality improvement program to identify and report problems, identify problem indicators, and perform chart audits and review.
Works with Case Manager to communicate patient status and problems daily.
Travel to patient homes and outreach locations to provide care under the direction of the Case Manager..
Accurately documents all patient visits on the day services were performed and on appropriate agency clinical forms.
Accurately reviews all verbal physician orders received by nursing personnel; provide authorization by signature on all verbal physician orders received prior to Patient treatment.
Follows up on reported or witnessed patient problems and concerns; maintains communication with physician, patient, family members, and case manager as necessary.
Rotation of call schedule as assigned.
Maintains compliance with all At Home Healthcare policies, procedures, and regulations, including all federal and state laws.
Maintains a clean and safe work environment.
Performs all other duties as requested by supervisor.
POSITION REQUIREMENTS, KNOWLEDGE, SKILLS AND ABILITIES:

Requires in-depth understanding of a comprehensive field of knowledge normally attained through a high school education and graduation from an accredited school of nursing. Requires current license as a Registered Nurse in the state of Texas, must maintain current license status.
Requires minimum of six months prior successful experience as a Registered Nurse. Home nursing care experience preferred.
Requires ability to speak, read and write English.
Must have and maintain current valid Texas Driver’s license and proof of current auto liability insurance.
Must maintain current certificate in basic life support (CPR).
Must submit to annual TB screening.
Requires travel to various geographic locations.
Requires the ability, willingness, and means to daily synchronize laptop computer with network server to ensure proper information exchange and to download software updates. Incumbent may synch from home, public Wi-Fi Hotspot, or work as the last official “act” of the day or
